Can a house slab be built with a step-down for an alfresco area?

Yes, genuinely common — a step-down (typically around 100mm) between internal living areas and an alfresco zone is a standard design feature, managing water ingress at the threshold while keeping the outdoor area at a slightly different level.

This is planned into the formwork from the outset, part of the normal design conversation with your builder or designer before the slab’s poured.
